How to Apply for a Passport

Passport applications are accepted by appointment only.

Applications are processed at the Gallatin location (Room 105) from 8:15 a.m. to 3:30 p.m. To schedule an appointment, call 615-452-4063.

What you will need to apply for a passport:

• Passport applications @ US Department of State (Having your application completed in black ink before you come into the office will save you time.)
• Please review proof of citizenship information. Applications without proper documents can not be processed.
• A passport photo. Each applicant will need to bring a passport photo. No photos are made at our facility.

NOTE: Department of State passport application fees will need to be paid by check or money order (and will be attached to each application.)
